Forms

All prospective users of either lab must:

1. Read and understand the Keck Lab Rules of Usage.
2. Complete, sign, and submit a JILA Keck Lab User Information Form. This form can be submitted in person or through email.
3. Non-CU users must also complete and sign the Services Agreement and provide a Purchase Order for billing usage and instruction.

Prospective users of the clean room must:

1. Complete the JILA Clean Room Safety Module and submit the Quiz

Users intending to use chemicals (in clean room lithography bay or in chemistry and dicing lab) must:

1. Complete the JILA Chemical Safety Module and submit the Quiz
2. CU users need to be current on their EH&S Lab Safety and Hazardous Waste Generator Training
3. Non-CU users will be given instruction in order to be in compliance with CU EH&S rules
4. Special training and arrangements must be made for use of Hydrofluoric Acid

Note: Once all forms have been submitted, arrangements need to be made with Keck Lab staff for a walk-through before access priveleges are granted.
